Telehealth consent
This telehealth consent is for care offered through Sunset Health, Inc. ("Sunset Health"), [BUSINESS ADDRESS]. It is separate from the terms of service. Contact support@sunset-health.com or [SUPPORT_PHONE].
What telehealth is
Telehealth uses electronic communications so a licensed clinician can evaluate you without an in-person exam. A visit uses secure messages, telephone, or video. The clinician tells you which method applies before the visit. Technology can fail, a message can be delayed, and a clinician cannot perform a hands-on exam.
Not for emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency department. Do not use this website or a text message to seek emergency care.
Clinician and location
The clinician is independently licensed. You must be physically located in a state where that clinician may provide care to you, and where Sunset Health offers services. Tell the clinician the state where you are located at the time of the visit. The clinician may be in a different state from you. The clinician decides whether telehealth is appropriate for your concern and whether any treatment is appropriate. A prescription is not automatic. The clinician may end the visit and direct you to in-person care.
Privacy and records
Protected health information is described in the Notice of Privacy Practices. The Privacy Policy describes this website. This consent does not replace the Notice of Privacy Practices. Medical records from the visit are kept for the period required by the state where care was provided.
Your choice
You may decline telehealth. Declining telehealth does not by itself waive a right to ask about other ways to receive care. If you continue a telehealth visit, you confirm that you have read this consent, that you can ask questions, and that you are in a state where the clinician may see you.
