Sunset Health

Privacy Policy

This privacy policy explains how Sunset Health, Inc. ("Sunset Health," "we," "us") handles personal information when you visit this website or communicate with us. Our business address is [BUSINESS ADDRESS]. Questions go to support@sunset-health.com or [SUPPORT_PHONE].

Who provides care

Sunset Health operates this website. Sunset Health, Inc. is the business you contract with for a membership. Sunset Health bills membership fees and clinician consult fees. Licensed clinicians provide the clinical services. A dispensing pharmacy, when one fills an order, is a separate licensed business and bills medication separately. This policy does not treat those roles as one entity.

Notice of Privacy Practices

Sunset Health, Inc. is the business that provides this membership and this website. The practice is the covered entity for care provided through this website. The practice's Notice of Privacy Practices for protected health information is at Notice of Privacy Practices. That notice is separate from this policy. It describes how the practice may use and disclose protected health information and the rights available under the HIPAA Privacy Rule, including the elements required by 45 CFR 164.520. This policy does not replace that notice.

Information we collect

How we use information

We use personal information to operate the site, respond to you, route a request for care to a licensed clinician, send messages you have agreed to receive, protect the site, and meet legal duties. A licensed clinician decides whether care is appropriate. Sunset Health does not make that clinical decision.

How we share information

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share it with licensed clinicians involved in your care, with a pharmacy when a clinician sends a prescription to be filled, with service providers that host the site or deliver messages under confidentiality obligations, and when the law requires it.

California privacy rights

If you are a California resident, the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA), gives you the right to know, correct, and delete personal information, and to appeal a denial of a request. You may use an authorized agent. We do not sell personal information. Send a request to support@sunset-health.com or [SUPPORT_PHONE]. We verify the request before we act on it.

Washington My Health My Data

If the Washington My Health My Data Act applies, we do not sell consumer health data. You may withdraw consent for collection or sharing of consumer health data, and you may ask us to delete it, by contacting support@sunset-health.com. We keep information when a legal duty requires us to keep it for care, security, or another legal obligation.

Text messages

If you give prior express consent, Sunset Health sends text messages about your care: appointment reminders and scheduling notices, prescription status notices, and account notices such as sign-in codes and security alerts. We do not send marketing texts under this program. Consent to receive texts is not a condition of purchase or of care. Message frequency varies. Msg & data rates may apply. Reply STOP to end messages. Reply HELP for help, or contact support@sunset-health.com or [SUPPORT_PHONE]. Mobile numbers and SMS opt-in data are not sold or shared with third parties or affiliates for their own marketing.

Security and retention

We use administrative, technical, and physical safeguards appropriate to the information we hold. Medical records are kept for the period required by the state where care was provided. Other personal information is kept only as long as needed for care, billing, disputes, and legal duties, and is then deleted or de-identified.

Your choices

You may ask to access, correct, or delete personal information we hold, and you may opt out of text messages by replying STOP. Contact support@sunset-health.com or [SUPPORT_PHONE] to make a request.

Children

This website is for adults 18 and older. It is not directed to children.
