Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This Notice of Privacy Practices describes how the practice may use and disclose protected health information and the rights you have. It includes the elements required by 45 CFR 164.520. Sunset Health, Inc. ("Sunset Health") is the business that provides this membership and this website. The practice is the covered entity for care provided through this website. Our business address is [BUSINESS ADDRESS]. Contact support@sunset-health.com or [SUPPORT_PHONE]. This notice is effective October 27, 2026.
Treatment, payment, and health care operations
The practice may use and disclose protected health information for treatment, payment, and health care operations.
- Treatment. We may share information with a clinician, a laboratory, or a pharmacy so you can receive care. For example, a clinician may send a prescription to a dispensing pharmacy.
- Payment. We may use information to bill a membership or a consult fee, to confirm payment, or to resolve a billing question. A dispensing pharmacy bills medication separately.
- Health care operations. We may use information to review the quality of care, to train staff, to contact you about an appointment, and to meet licensing and compliance duties.
Other uses and disclosures
The practice may also use or disclose protected health information without your written authorization when the HIPAA Privacy Rule allows it, including when the law requires the disclosure, for public health activities, for health oversight, for judicial and administrative proceedings, for law enforcement in the circumstances the Privacy Rule describes, to avert a serious threat to health or safety, for workers' compensation when the law allows it, and for specialized government functions when the Privacy Rule allows them.
The practice may contact you with appointment reminders and other messages about your care.
Authorizations
Other uses and disclosures of protected health information are made only with your written authorization. You may revoke an authorization in writing. A revocation does not apply to the extent the practice has already acted on the authorization. The practice does not sell protected health information. A use or disclosure that the Privacy Rule treats as a sale, or as marketing that requires authorization, is made only with your written authorization.
Your rights
You may ask the practice to do the following. Contact support@sunset-health.com or [SUPPORT_PHONE].
- Inspect and obtain a copy of the protected health information the practice keeps about you, with the exceptions the law provides, and ask that the copy be sent to another person.
- Ask the practice to amend protected health information if you believe it is incorrect or incomplete. The practice may deny a request in the circumstances the Privacy Rule allows, and it will explain the denial.
- Receive an accounting of certain disclosures of protected health information.
- Request a restriction on a use or disclosure for treatment, payment, or health care operations. The practice is not required to agree to every restriction. When you pay in full out of pocket for an item or service, you may ask the practice not to share information about that item or service with a health plan for payment or health care operations, and the practice will honor that request unless a law requires the disclosure.
- Request confidential communications, such as contact at a different address or phone number. The practice will accommodate a reasonable request.
- Obtain a paper copy of this notice on request, even if you agreed to receive it electronically.
- Receive notice of a breach of unsecured protected health information as required by 45 CFR 164.404. The practice will notify you without unreasonable delay and no later than 60 calendar days after it discovers the breach.
Our duties
The practice is required by law to maintain the privacy of protected health information, to provide this notice of its legal duties and privacy practices, and to abide by the terms of the notice that is currently in effect. The practice may change this notice. A revised notice applies to information the practice already has and to information it receives later. The revised notice will be posted on this page with a new effective date.

You may complain to the practice and to the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe your privacy rights have been violated. A complaint to the practice goes to support@sunset-health.com or [SUPPORT_PHONE]. A complaint to the Office for Civil Rights may be filed at https://www.hhs.gov/hipaa/filing-a-complaint/index.html.
